  • Diseases and Illnesses

    The childcare manual (linked in Quick Links box) includes information about specific diseases and illnesses that affect school attendance. It will give guidelines on when your child is ready to return to school.

    If your child has asthma, please complete the Asthma Action Plan whether your child uses an inhaler at school or not. Please send the completed form to the health office in your child's school.

    Emergency Contact Information

    Schools need your updated contact information in the event of illness or injury at school.

    Emergency cards can be completed and updated at any time. Call your child's school to update this information or send this completed form to school with your child. Or you can update all emergency contact information on Family Access.

    Immunizations Required for School

    According to Minnesota Statute 121.A15, students need immunizations for enrollment in schools. Updated immunizations are required upon entering Kindergarten and Grade 7. Other non-required immunizations may be given during the appropriate age level.

    No student is required to receive an immunization which is contrary to the conscientiously held beliefs of the parent or guardian. To receive an exemption, complete the exemption portion of the immunization form, have it notarized, and return it to school.

    Medication at School

    Medication will be administered during school hours when necessary. Medication includes both prescription and non-prescription (over-the-counter) medication.

    If your child requires medication at school, please note that the medication can be administered only when a release form (signed by both the parent and the health care provider) is on file in the school health office.

    Severe Allergy/Anaphylaxis Form

    If your child has a severe allergy that requires medication and/or an epi-pen, please complete the allergy action plan form. Return it to the school's health office with the medication your child may need in the event of exposure to an allergen.
