For grades 7-12, submit 1 packet per school year for participation in athletics. There are 8 signatures required for this packet. |
| 2. Fine Arts / Club Eligibility Packet (6 pages) |
Submit if participating in a fine arts or club program. Submit even if you are participating on an athletic team. There are 5 signatures required for this packet. |
| 3. Renewal Form |
For grades 7-12, submit for the 2nd or 3rd sport, activity, or club of the year. Use this form only if you have already participated in a sport, activity, or club this school year (2009-2010) and have a packet on file. |
| 4. Physical Exam Form |
Physical exams are good for 3 years - see information below. Print the form and bring with you to the physical appointment. |

Athletic, Fine Arts, and Club Fees
Cash, check made out to ISD #277 or on-line payment (click on link above) is due at the time of registration according to the schedule below:

Note that checks for Activity Fees are usually deposited 4 to 8 weeks after the first week of practice.
$150.00 Damage Deposit - Athletics
Separate check made out to ISD #277 is required of all athletes regardless of sport. Deposits are retained for one academic year and shredded after the completion of the spring sports season provided that all uniforms and equipment are returned to the school district in good condition. If any school district property issued to the student athlete is not returned, or is returned damaged, the district will exercise its prerogative to cash the damage deposit in order to replace lost or damaged property. Furthermore, if the $150.00 deposit does not cover replacement costs, you will be billed the difference.
Current Sports Qualifying Physical
A physical that has been done within the last three years must be on file with the Activities Office. If your physical will expire during the sports season for which you are registering, you must provide a current physical BEFORE you can begin practice. Physicals done on or before the dates below are considered expired for the 2009-2010 school year as outlined:
October 31, 2006—expired for Fall 2009
March 31, 2007—expired for Winter 2009-2010
May 31, 2006—expired for Spring 2010
